Known/Documented Data Issues:
1) For the timeframe of 2011-10-01 to 2011-03-30, values were associated with the closed MetroSouth Hospital (entity_id 106840) that should have been attributed to the open one (entity_id 101212). This is because these two hospitals shared NPI_ids and mpn_ids
Resolutions to Data Issues:
1) Data for the closed hospital was duplicated and inserted into the entity_values table. Because closed hospitals do not display, the previous extra rows did not need to be removed
